Who qualifies for Home Health Care?
Home health care services are eligible for people of any age and medical situations! Not for just the elderly. Home health care can be for you if are home bound and require medical care or threat that you are incapable to receive through a family member. You may qualify if you:
■ Have recently and/or frequently been hospitalized.
■ Have a knowledge deficit pertaining to disease process, medication, and/or diet.
■ Have endured an injury or illness that requires additional treatment.
■ Have a medical condition that needs frequent assessment/monitoring such as diabetes, stroke, lung disease, heart disease, or a chronic illness (such as multiple sclerosis).
■ Has the need for one or more skilled services including Skilled Nurse or Rehabilitation Services.
■ Suffers from bedsores or wounds that do not seem to improve or heal.
■ Remains under the care of a Physician authorized services.
■ Has a new or exacerbated disease process.
Who is eligible?
In order to be eligible for our services, you must meet the following conditions:
■ Medical necessity as defined by Medicare/Medicaid, and other insurances.
■ Must be under the care of a physician.
■ Must be homebound or unable to ambulate independently.
What about Medi-Cal?
Some extra aid may also be available from Medi-Cal for those who qualify.
Private insurance is accepted.
Patients may have health insurance with a private insurance company. The benefits may vary, depending on what policy the private insurance company provides. We will do our best to clarify the coverage and work with the patient to maximize the benefits.
